How to Complete the Contact Log for Student Teachers

Use this log to track communication you've had with your student teacher. You will be able to edit and add more items to this log until you are completely finished with the field experience.

NOTE: Students will not be able to view the information you add to the form.

  1. Login to ePortfolio
  2. Enter your NetID and password.
  3. Click your name in the Utility bar, then select the ED/OEP/Supervisor role from the drop-down menu.
  4. Click the student's name.
  5. Click the Student Teacher Contact Log.
  6. Enter the date the student teacher began student teaching.
  7. Enter the Student Teacher's TEA ID number.
  8. To begin entering the log, click the Add Row icon [+].
  9. Choose the activity type from the activities menu.
  10. Enter the date of the activity.
  11. Enter the start and ending time.
  12. Enter brief comments about the event.

    NOTE: Make sure you have no spaces in the text box before or after your comments.
     
  13. Repeat steps 8-12 for each event you need to record.
  14. Complete the appropriate statementsdates, attendance logs, and enter other required information.
  15. You may attach one supporting document, if needed. 

    NOTE: Supporting documents are not required. If you feel you need to upload a supporting document, please contact the Director of the Office of Educator Preparation to be sure it is necessary. Supporting document examples may include a Word document of all email communication between a difficult student teacher and the Student Teaching Faculty.

    NOTE: You may leave the grade field blank, as grades are not applicable.
     
  16. Click Save at the bottom of the screen to save your work and continue working. Click Complete to save your work and exit the screen.
