Student Teaching Faculty Checklist

Before You Begin

The following is a checklist of all items that must be completed by Student Teaching Faculty and Teaching Intern Supervisors to successfully submit each Student Teaching or Teaching Intern Binder. 

NOTES:

  • If your Student Teachers are seeking All-Level certifications, generally enrolled in 4380 and 4381, or are completing two placements for another certification, you will most likely have two binders to complete - one for each placement. These are slightly different from the Full-Term binders.
  • Teaching Interns will use the same binder both semesters of their internship.

Select your Binder Type:


Full-Term Placements

For each Full-Term Placement binder, you will:

  1. Complete the Contact Log.
  2. Complete all six observation forms
    • Observations 1, 2 and 3 are mandatory for every binder, every time. Every Field is Required.
    • Observations 4 through 6 are optional, and if you are not completing the observation, you'll open the form, select No, then click Complete.
  3. Complete the Midpoint Evaluation 
  4. Complete the Principal Signature Form. You can upload this as almost any file type, including a photograph or scanned document.
  5. Complete the Student Teaching Improvement Growth Plan.

    NOTE: If you are not completing a Growth Plan, you can select No, then click Complete.
     
  6. Complete the Final Evaluation.
  7. Click Submit.

    CAUTION: You are ready to Submit when there are no more red pending flags in the Field Experience Forms area.

    Congratulations! You have successfully submitted a Student Teaching Binder.

Split Placements

For each Split-Placement binder, you will:

  1. Complete the Contact Log.
  2. Complete all four observation forms
    • Observations 1 is mandatory for every binder.
    • You must complete Observation 2 in one of the two binders.
    • All Fields are required.
    • Observations 3 - 4 are optional
      • You will still open each form, select No for the ones you're not using, then click Complete.
  3. Complete the Principal Signature Form. You can upload this as almost any file type, including a photograph or scanned document.
  4. Complete the Student Teaching Improvement Growth Plan.

    NOTE: If you are not completing a Growth Plan, you can select No, then click Complete.
     
  5. Complete the Final Evaluation.
  6. Click Submit.
